We implement customized care plans provided by Department of Health and Senior Services that provide the right mix of services to preserve your loved ones’ independence, maintain in-home safety, and enhance quality of life:
CONSUMER DIRECTED SERVICES
Consumer Directed Services (CDS) is a Medicaid based program which provides personal care attendant (PCA) services to individuals with disabilities; enabling them to live independently. Individuals in the CDS program are trained to hire their own attendants to assist them with daily tasks.
The Superb Caregivers CDS program is a consumer controlled model. Consumer control means that the person with a disability receiving services is the employer of the attendant. Program participants are responsible for locating, hiring, setting work schedules and managing their own attendants. A friend or family member may be hired as an attendant, with the exception of a spouse or anyone under the age of 18. The attendant must have a clean background check through the Missouri State Family Care Safety Registry before being employed by a CDS participant. Superb Caregivers provides training on hiring and managing attendants for CDS participants in St.Louis County and St. Louis City
Duties Include but are not limited to:
- Light Housekeeping Chores
- Dressing and Hygiene Care
- Cooking, Preparing Meals & Feedings
- Transportation and Errands
- Relief Services
- Nursing Services
- Part Time Live-In Services
- Weekly medication set up and monitoring
Client Advocacy/Care Management
Superb Caregivers can provide written updates on client progress for attorneys, trust officers, powers of attorney, and designated family members. Getting an update about your loved one is only a phone call away no matter where you live. This service provides the total supervision that many clients need to remain at home safely.
*Mental Healthcare Personal Assistance
Superb Caregivers also provides personal assistance and Registered Nurse visits to mental health clients. The duties may vary based on the client plan of care needed.
Nursing Services
Our highly skilled RN’s and LPN’s assist with medication management, wound care, pain management, and extended nursing care. They can also coordinate the client’s care with other members of their healthcare team, including accompanying clients to physician appointments or procedures.
In Home Health Aides
Home Health Aides provide hands on services such as bathing, feeding, respite care, medication reminders and are supervised by a registered nurse, and other home care services as listed under Consumer Directed Services. Home Health Aides can also take vital signs and assist clients with transferring into/out of bed or moving about the home.
Private Pay Services
Private Pay Services varies, they are customized based on the client and may provides a wide range of assistance with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, assisting with ambulation or transferring, toileting, feeding and providing medication reminders. In addition, instrumental activities of daily living (IADLs), such as shopping, meal preparation, making medical appointments, transportation, laundry and companionship.
**Adult Brain Injury Program
Adult Brain Injury program provides assistance to adults ages 21- 65 who have suffered a traumatic brain injury. The purpose is to provide support that will enable them to return to a productive lifestyle in their community.
